If you pay by mail, please fill out the registration, print and mail to Betsy. You will receive the mailing address after registration.

To use PayPal after registration, go to PayPal.com and click on the personal tab, then on Send Money.
Fill in the email address -betsym (at) 1starnet.com and follow the directions.

After you submit the form, you will see a page which has form submitted at the top. Click on continue.

You have a choice of a Traditional Box Lunch including chips, pickle spear & a chocolate chip cookie OR the French Box Salad plus a chocolate chip cookie. Please choose ONLY the Traditional Box Lunch OR the French Box Salad.

.Traditional Box has a choice of a 1/3 lb. traditional sandwich with lettuce and tomato.
